Eduardo Bruera clarifies the challenge, which is to control many different treatment and illness induced symptoms in the single patient. The challenge is less so the disease, but the patient itself. One needs to understand what issues are important, what is causing patient and family suffering. In the end, all comes together in the patients’ brain, so you could say the disease talks to the brain. Important in all this is the proper assessment of (the severity of) symptoms. Different approaches to this are in use. Usually the patient assesses the symptoms as more severe than the physician does.
